Creating forms

Learn how to create a form:

  • Navigate to the "Form builder" page under Settings.
  • Click on "Create a new form."
  • Enter the name of your form. This form name will be publicly visible to your visitors who fill out this form.
  • Optional: Include a description below for your form. This description will also be publicly visible to your visitors.
  • Click on "Add a field to this form" and select an existing LACRM field to add to your form.
    • Note that rather than creating a new form field, you’re linking existing LACRM fields to your form. This allows data to go directly into the field in LACRM.
    • You can also change the name of an existing LACRM field in your form to make it more specific to the form. For example, instead of asking your visitors to select a dropdown option for a "Source" field, you can rename that field to "How did you hear about us?" for just this form. Your visitor will then select a value from your existing "Source" field options, and what they submit will populate your "Source" field.
  • Add content blocks to segment your fields within the form or to include additional text. Click on “Add a field to this form” and select “A field not in your CRM.”
    • Choose “Section header” to segment your fields. Enter a section name, which will appear in larger text, and optionally enter an additional description. If necessary, click and drag the section header to the desired location in your form.
    • Choose “Paragraph text” to add a block of text. Enter the desired text, including any formatting you’d like. If necessary, click and drag the paragraph text to the desired location in your form.
    • Note that content blocks are not included in the "All submissions" tab or in exports of form submissions.
  • Once you have added all the fields you need for your form, click on "View form" in the top right corner to see the public version of your form.
